How can I have my medical records transferred to Dr. Tanner?
That's easy and simple with an "Authorization for the Release of Dental Records" form. Just choose one of the release forms below to complete and give to us. We'll contact your former doctor(s) and take care of the rest.
Both forms below are the same but they can be filled in differently: one is digital and the other is for filling by hand. Whichever one you choose needs to be printed and handed to Dr. Tanner's office. Let us know if you have any questions.
